
Many patients with dry eye symptoms do “all the right things”—using artificial tears, warm compresses, even changing their environment—yet still struggle with burning, redness, blurry vision, or a constant gritty sensation. The source of this discomfort is not always directly the tear film or ocular surface, but the eyelid margin itself. Demodex blepharitis is a common cause of flakey, debris-like material that can build up on the eyelid margin/lash line. Demodex are skin mites that are harmless in normal concentrations, but overgrowth of these mites along the eyelid margin can cause many classic dry eye symptoms such as burning, irritation, dryness, and crusting.
Due to the presence of demodex, the eyelashes naturally develop a biofilm—a thin, sticky layer made up of bacteria, oils, dead skin cells, and debris. Research and clinical observation suggest this biofilm can mature and reorganize approximately every 60–90 days, especially along the lid margins and around the base of the eyelashes. While some level of bacteria on the skin is normal, problems arise when this biofilm becomes dense and excessive, leading to a cascade of ocular problems.
As the biofilm builds up, it can interfere with the meibomian glands, which are responsible for producing the oil layer of the tear film. These oils slow tear evaporation and keep the eyes comfortable. When bacteria within the biofilm release inflammatory byproducts—such as lipases and toxins—they alter the quality of meibomian gland secretions, making them thicker and unable to secrete properly. The result is unstable tears, faster evaporation, and worsening dry eye symptoms that can drastically decrease quality of life.
Because the eyelid biofilm continually regenerates, occasional cleaning isn’t enough.
